Resume (must show at least 1 year’s work experience in target specialty)
2 professional references from manager or supervisor from nurse’s most recent experience on resume
Diploma or transcript

Credentials:

Driver’s License
LVN License
BLS (issued by American Heart Association)
Fire Card (for LA County facilities)
MAB or CPI

Medical:

Physical from the past year – If you do not have a recent physical, you may bring this form to a physician.
Negative 2-step PPD or Quantiferon form the past year OR Positive PPD and clear chest X-ray
Flu shot (can be declined for most facilities)
Covid vaccines with booster (can be declined for most facilities)
MMR (positive titer or proof of 2 vaccines)
Varicella (positive titer or proof of 2 vaccines)
Hepatitis B (positive titer or proof of vaccine series)
Respiratory Fit Test
