Experience & Education

  • Resume (must show at least 1 year’s work experience in target specialty)
  • 2 professional references from manager or supervisor from nurse’s most recent experience on resume
  • Diploma or transcript

Credentials:

  • Driver’s License
  • RN License
  • BLS (issued by American Heart Association)
  • Fire Card (for LA County facilities)
  • ACLS (issued by American Heart Association)

Medical:

  • Physical from the past year – If you do not have a recent physical, you may bring this form to a physician.
  • Negative 2-step PPD or Quantiferon form the past year OR Positive PPD and clear chest X-ray
  • Flu shot (can be declined for most facilities)
  • Covid vaccines with booster (can be declined for most facilities)
  • MMR (positive titer or proof of 2 vaccines)
  • Varicella (positive titer or proof of 2 vaccines)
  • Hepatitis B (positive titer or proof of vaccine series)
  • Respiratory Fit Test
