Experience & Education

Resume (must show at least 1 year’s surgical tech experience in an acute care hospital or surgical center within the past three years)
2 professional references from manager or supervisor from most recent experience on resume
Graduate of a CAAHEP accredited Surgical Technology program

Credentials:

Government-issued photo ID (Driver’s License or Passport)
Current and valid Certified Surgical Technologist (CST) credential from the NBSTSA
BLS (issued by American Heart Association)
Fire Card (for LA County facilities)

Medical:

Physical from the past year – If you do not have a recent physical, you may bring this form to a physician.
Negative 2-step PPD or Quantiferon form the past year OR Positive PPD and clear chest X-ray
Flu shot (can be declined for most facilities)
Covid vaccines with booster (can be declined for most facilities)
MMR (positive titer or proof of 2 vaccines)
Varicella (positive titer or proof of 2 vaccines)
Hepatitis B (positive titer or proof of vaccine series)
Tetanus/Diphtheria/Pertussis (Tdap, positive titer or proof of vaccine)
Respiratory Fit Test
