Experience & Education

  • Resume (must show at least 1 year’s surgical tech experience in an acute care hospital or surgical center within the past three years)
  • 2 professional references from manager or supervisor from most recent experience on resume
  • Graduate of a CAAHEP accredited Surgical Technology program

Credentials:

  • Government-issued photo ID (Driver’s License or Passport)
  • Current and valid Certified Surgical Technologist (CST) credential from the NBSTSA
  • BLS (issued by American Heart Association)
  • Fire Card (for LA County facilities)

Medical:

  • Physical from the past year – If you do not have a recent physical, you may bring this form to a physician.
  • Negative 2-step PPD or Quantiferon form the past year OR Positive PPD and clear chest X-ray
  • Flu shot (can be declined for most facilities)
  • Covid vaccines with booster (can be declined for most facilities)
  • MMR (positive titer or proof of 2 vaccines)
  • Varicella (positive titer or proof of 2 vaccines)
  • Hepatitis B (positive titer or proof of vaccine series)
  • Tetanus/Diphtheria/Pertussis (Tdap, positive titer or proof of vaccine)
  • Respiratory Fit Test
