The forms listed here will be transferred to the new system and removed from this page by the end of the month.

Throughout the rest of 2026, we will be rolling features, including online hous reporting, and a dedicated mobile app for booking!

  • USC Arcadia Hospital
    • Parking is free
  • USC Keck Medical Center
    • Parking is $20
    • O’Neal Lot in the pay by plate where parking is $2/hour – max $14, however at Keck it would remain at 2-hours free with $20/max.
    • Keck- Parking is in P6- access the parking structure off San Pablo Street.
    • They do not have a specific parking location for the registry.  If you would like the closest, it is right next to the hospital for a fee.
    • KECK does not validate.
  • USC Verdugo Hills Hospital
    • Parking is free
Other Requirements & Notes
  • California Sexual Harrassment Prevention Training
  • Name and phone of the clinic must be on medical documents
  • If positive for PPD, Chest X-ray must be completed within 1 year for Keck/Norris
  • If positive for PPD, Chest X-ray must be completed within 5 years for Verdugo
  • Bring driver’s license on 1st day
  • 10 panels within a year – Effective 1/1/2024 THC should not be tested in drug screen. – need proof expanded Opiates were tested
  • Digital self-picture (square, high resolution, white/off-white background, no filters)
